WHAT TO KNOW
• Flu vaccine: $25 for uninsured patients
• COVID vaccine: $110 for uninsured patients
• Pre-register by scanning the QR code below (not required)
BRING WITH YOU:
• A copy of your insurance card is required
• If you have Medicare, please have both your Medicare card and Medicare Advantage card available, if applicable.
